Community Health Nurses (CHNs) provide a comprehensive range of core nursing functions and services (community/public health, primary care, and home care) in our Indigenous Communities. CHNs focus on promoting health, preventing disease and injury, protecting population health, as well as a focus on curative, rehabilitation, and supportive or palliative care.

Home and Community care is support for community members with acute, chronic, palliative, or rehabilitative health care needs. Helping community members live safely and independently in their own home for as long as possible.
Home care services are designed to compliment and supplement, but not replace your efforts to care for yourself with the assistance of your family, friends or other community members. Home and community care services are available to assist you on a short-term or long-term basis depending on your care needs.
